Holistic Assessment: Either completed independently or with a VigR Health navigator, the holistic assessment helps us evaluate a person’s overall wellness and establish a baseline across physical, cognitive, mental/emotional, and environmental factors. Issues in any of these interrelated areas can signal the potential for decline. When repeated over time, a holistic assessment can reveal subtle changes that indicate the need for early interventions.
Clinical Assessments: When holistic assessments uncover potential risks, our navigators use evidence-based standardized and proprietary tools to explore specific areas of concern.
Validated Recommendations
Armed with complete, accurate assessment data, our wellness navigators:
- Implement interventions to mitigate immediate risks
- Develop individualized healthy aging plans, including lifestyle, environmental, and behavioral changes to extend health span
- Maintain ongoing contact with your policyholders, members, or residents
Falls Risk: A Common Example
According to the CDC, falls are the leading cause of injury among adults aged 65 and older. Balance plays a role, but, from a holistic standpoint, it’s only one of many, often less obvious contributing factors. Based on best-in-class strategies, our falls-risk recommendations may include:
- Medical evaluations to uncover underlying issues
- Balance-improving practices such as Tai Chi
- Weight-bearing and strength exercises
- Physical therapy
- Assistive devices (e.g., canes) for high-risk individuals

Using motivational interviewing (MI) to understand each person’s unique situation, incentive, and readiness for change, VigR Health navigators introduce the most appropriate, customized recommendations with the highest likelihood of success.
In the falls-prevention example, a VigR Health navigator might help a given individual find a gentle yoga class, connect with a personal trainer, declutter their living space, and access curated resources such as SilverSneakers® and VigR webinars.
Follow-up
Risk assessments and recommendations are just the first part of the process. Follow-up is essential to maintaining rapport and ensuring participation. Regular outreach can also significantly improve a person’s satisfaction with their long-term care provider.
